STEP 1: Follow the directions below. Collect your hair, nail, urine, and saliva samples and place them together in one plastic zip lock bag.

• 1 TSP HAIR- This can be collected from anywhere on the body or from your brush. If you are collecting from your brush please make sure to clean out the old hair and only provide us with the newest strands.

• 5 Nail Clippings - Toes or Fingernails fine.

• 1 Q-tip Saliva sample - Swab your mouth in the AM before brushing your teeth.

• 1 Q-tip with Urine sample - Pee on one end of the Q-tip first thing in the AM.

STEP 2: In Sharpie write your NAME, BIOLOGICAL SEX, DOB, and PRACTITIONER NAME on the bag.

STEP 3: Send your samples in a USPS Priority envelope. Keep your receipt for tracking. It takes an average of 3 weeks for processing. Please don't require a "signature: when you send your samples. It will delay your scan results.

STEP 4: Please fill out your intake as soon as possible, failure to do this will delay your results. This should have been sent to your email directly after purchase.
